Directions to CareLink Office:

Heading South on Route 95: take Thurbers Avenue Exit 18, bear left off exit, then take right onto Allens Avenue. Chapman Street is ½ mile up on the right. Continue 2/10 mile on Chapman - to #225 (on the right, the only 4 story building). Parking is located across the street or behind the building. CareLink is located on the 1st floor.

Heading North on Route 95: take the Eddy St/Allens Avenue Exit (#19), bear right towards Allens Avenue, take a right at the lights. Chapman Street is ½ mile up on the right. Continue 2/10 mile on Chapman - to #225 (on the right, the only 4 story building)
