NRI Community Services has developed a multi-disciplinary Mobile Geriatric Assessment Team to provide psychogeriatric consultation, education and wellness services to residents of both local and statewide senior housing facilities. Services are designed to assist nursing facility
staff with challenging resident issues, which go beyond the level of services generally provided, by the facility staff. The goal of the service is to address the presenting resident concerns while providing the nursing facility staff with effective interventions and skills for dealing with future resident issues.

Mobile geriatric team members in collaboration with the Emergency Services Department provide immediate and assertive outreach, evaluation and crisis intervention services to seniors presenting in psychiatric distress. Such services are provided at the Center via face-to-face, telephone contact, in homes, senior housing, or other locations as the situation demands. This Team also functions as a consultation and training resource for other community agencies and organizations.


CRA, Inc.

CRA, Inc., an affiliated corporation of NRI Community Services, has a regional contract with the Department of Elderly Affairs and operates exclusively to provide home and community based casework, referral and advocacy services for elders living in the South County and Coventry areas. A nurse heads a team of Bachelor-level casemanagers who assist seniors in accessing appropriate supportive services to include:

NRI Report Card

EVALUATION OF SERVICES:

Annually, every consumer enrolled in NRI Community Services' Community Support Program for individuals with severe and persistent mental illness is offered an opportunity to provide anonymous feedback about the behavioral healthcare services they receive through the Agency.

Consumers are surveyed using the Rhode Island Outcome Evaluation Instrument (RIOEI), which was created collaboratively by the:

· Department of Mental Health, Retardation and Hospitals (DMHRH) Division of Integrated Mental Health Services (DIMHS)
· Community Mental Health Organizations
· Consumers
· Advocates
· Additional key stakeholders.

The RIOEI incorporates the Mental Health Statistical Improvement Project (MHSIP) Survey, a nationally used instrument developed by and for consumers and their advocates.

CONSUMER FEEDBACK:

Since implementation of the RIOEI in October 1998, consumers of NRI Community Services consistently respond favorably to all four domains

· Access
· Appropriateness
· Outcomes

REFERRAL SOURCE SATISFACTION:

Often, someone other than the consumer initiates the request for services. NRI Community Services has developed relationships with numerous agencies who contact us when there is concern about an individual's mental health or substance abuse. Here's how some of those referral sources responded in a recent "Referral Source Satisfactions Survey" conducted by Carelink.

· 100% would make future referrals to NRI Community Services.

RESULTS OF THE DEPARTMENT OF MENTAL HEALTH, RETARDATION, & HOSPITALS ANNUAL RECORD REVIEW:

Each year, the DMHRH, DIMHS visits every Community Mental Health Center throughout the State in order to review record-keeping practices in several areas, including:


· Medical
· Substance Abuse
· Employment
· Recovery
· Social Integration

In each of the areas shown below, NRI Community Services exceeds the State average, reflecting the Agency's efforts to provide the highest quality behavioral healthcare services.



Medical Needs:

The provision of high quality mental health services is dependent upon many factors, including coordination of physical health care. By understanding the kinds of physical health problems of each consumer and working with primary care physicians, NRI Community Services can help consumers decide which services are just right for them Not only will this get the consumer on the road to recovery more quickly, it can prevent the development of serious medication side effects or interactions.



Substance Abuse:
NRI Community services is committed to providing fully integrated mental health and substance abuse services. Full integration enhances recovery and improves outcomes.


Employment:
Sustaining paid integrated community employment offers an array of benefits, both direct and indirect. These include increased financial resources, health benefits, and paid vacation time. Many consumers who are employed experience improvements in their level of social integration, self-esteem, and overall satisfaction with their place in the community.



Recovery:
Recovery implies different things to different people. On a tangible level, it may be achieved through success in the previously noted areas - stabilization of medical conditions, remaining abstinent, or enjoying one's job. Harder to measure, but equally important, are the more intangible elements of self-direction, improved self-worth, more fulfilling social interactions, and so on.
